The thought is for this form to be filled out once the sales rep believes the customer is signing up and no longer needs any follow-up.
The form would go to the Startup Coordinator who would then have all notes and information laid out in a clean format.
The Startup Coordinator could use that info before or during the startup process.
If the lead doesn't sign up within (1 or 2 weeks - discussion point), then the Startup Coordinator will reach out to that Sales Rep to follow-up with the lead.
